Acute Otitis Media ( AOM ) - Ear Pain

Acute Otitis Media ( AOM ) or Middle Ear disorder or Ear Pain characterized by Moderate to severe onset middle ear  sever pain, Ear effusion, Physical evidence of the middle ear inflammation.

Causes :

Childhood bacterial infection
Cold
Flu
Sinus Infection
Allergies
Sore throat
Respiratory Infections.

Symptoms :

  • Ear pain ( Less than 48 hours )
  • Swelling
  • Edema
  • Pus Discharge form ear
  • Redness
  • Fever
  • Irritability
  • Bulging of tympanic membrane

Risk Factors :

Age : Younger
Upper respiratory tract infections
Family history of recurrent acute otitis media
Immunodeficiency
Allergies
Exposure to environmental smoke
Respiratory Irritants
No breast feeding
Pacifier use
Craniofacial abnormalities
Gastroesophageal reflux

Diagnosis :

1) Physical Examination : Using otoscopy visualized the ear parts.
2) Symptomatology : Without otoscopic findings of inflammation.
3) Investigations :
     Most cases can be diagnosed clinically. Such as
  • FBC
  • CRP
  • Blood Culture  and Fluid culture : If any discharge present and if any signs of sepsis. Fluid culture has to be taken before starting Antibiotic Therapy

Treatment

Diagnosis established by physical examination findings and presence of symptoms

Antibiotic Therapy :

First line Antiboitics
Initially firsit line antibiotics to be start with high dose.

1. Tab : Amoxycilin 625mg
Adult Dose : 625 mg in two times per day for 3 days or 5 days or 7 days or 10 days
Note : Before taking check for allergies.

2. If symptoms not cured after 3 days of treatment administer ceftriaxone, clindamycin, tympanocentesis

Children Dose : 90 mg per kg ay in two divided doses. It is the first choice for initial antibiotic therapy in Children with AOM.

Second line Antiboitics Therapy

2. Ciprofloxocin Ear Drops
Dosage : Ciprofloxocin Ear drops 3 times or 4 times per day
Note : Before taking check who are allergic to pencillin.

Pain Management :

Adequate analgesia. Use NSAIDs
Analgesics are recommended for symptoms of ear pain, fever and irritability. Analgesics are particularly important at bedtime ( Due to pain disrupted to sleeping pattern )
1. Acetaminophen
2. Ibuprofen
1. Ketrol DT
2. Paracetamol

Note : Check for Kidney disease

Swelling :

1. Dexamethasone ear drops

Procedure : 

Ear Irrigation : Ear irrigation is a procedure to clear the fluids of inner ear. It is performed by the physician.

Difference between Cetrazine and Levocetrazine

Cetrazine and Levocetrazine both are Antihistamine Drugs also called Antiallergic Drugs. These drugs are used to relive the allergic symtoms such as watery eyes, runny nose, Itching nose and eyes, sneezing, hives. These drugs works blocking by certain natural substance ( histamine ) that the body makes during an allergic reaction.

Levocetrazine is less sedative than Cetrazine. 5mg of Levocetrazine dose is equal to 10mg of cetrazine. These medications are to be taken during night hours before sleeping to work effectively.

Electroconvulsive Therapay-ECT

ElectroconvulsiveTherapay (ECT) is a procedure in which electric currents are passed through the brain, deliberately triggering a brief seizure. Electroconvulsive therapy seems to cause in brain chemistry that can immediately reverse symptoms of certain mental illnesses. It often works when other treatments are unsucessful.

          Nurses have an important role to deliver when a client is to undergo Electroconvulsive Therapy. Find out what are the responsibilities and activities of the nurse during electroconvulsive therapy.

Emotional and Educational support to the Client & Family

  • Encourage the client to discuss feelings, including myths regarding ECT.
  • Teach the client and the family what to expect with ECT.

Pre treatment Protocol for ECT :

  • Ascertain if the client and the family have received a full explanation, including the option to withdraw the consent at any time.
  • Withhold food and fluids for 6 to 8 hours before treatment.
  • Remove dentures, glassess, contact lenses,  hearing aids, hair pins and etc.
  • Have client void before the treatment.
  • Give preoperative medications as ordered.
    - Atropin : To prevent potential for aspiration and to help minimize brady arrhythmias in response to electrical stimulants. Instead of Atropin Glycopyrrrolate ( Robinul ) can be used.

Nursing Care During Procedure :

  • Place a blood pressure cuff on one of the clients arms.
  • As the intravenous line is inserted and EEC and EEG electrodes are attached, give a brief explanation to the client.
  • Put on the pulse oxymeter to the client's finger.
  • Monitor blood pressure throughout the treatment
  • Medication to be given
    - Short Acting Anesthetic ( Brevital )
    - Muscle relaxant ( Succinylcholine )
    - 100% oxygen by mask via positive pressure.
  • Check if the bite block is placed in prevent biting of the tongue.
  • Electrical stimulus given ( Sezure should last 30-60 sec ).

Post treatment Nursing Care :

  • Have the client go to a properly staffed recovery room
  • Once the client is awake, talk to the client and check the vital signs.
  • Give frequent orientation and reassurance to allay confusion.
  • Check the gag reflex before giving client fluids, medications or breakfast.
